Your rights, risks, and responsibilities when receiving psychiatric evaluation, psychotherapy, and medication management via telehealth in Massachusetts.
Purpose and Nature of Treatment.
Explanation of evaluation, psychotherapy modalities (CBT, SFBT, Positive Psychology, Motivational Interviewing), and medication management.
Risks/Benefits and Alternatives.
- Potential benefits (symptom reduction, improved functioning) 
- Potential risks (side effects, emotional discomfort) 
- Alternatives (referrals, different modalities) 
Patient Responsibilities.
- Provide accurate information 
- Attend appointments and follow safety plans 
- Inform us of medication side effects or changes 
Confidentiality and Limits.

- Limits: danger to self/others, abuse/neglect reporting, court orders 
Consent to Coordinate Care.
You may authorize us to communicate with your PCP/therapist/others via a Release of Information (ROI).
Medication & Controlled Substances Policy
Our prescribing practices, refills, PDMP checks, and safe‑use expectations for psychiatric medications in Massachusetts.
- Prescribing decisions are individualized and clinically indicated. 
- No early refills, lost/stolen replacement, or sharing of medications. 
- Urine drug screening and treatment agreements may be required when clinically appropriate. 
- We may check the Massachusetts Prescription Monitoring Program (PMP) prior to prescribing controlled substances.
